Step 2: Submit Required Forms
In order to comply with Federal and State laws we require you complete, sign and submit three forms:

Complete Audiogram: A complete Audiogram* (hearing test) is necessary to properly program your digital hearing aids to your specific hearing loss. The U.S. Food and Drug Administration mandates that you have a complete hearing test within six (6) months of the date of purchase of a hearing instrument. Medical History & Medical Waiver:
We strongly recommend that you see an Ear, Nose and Throat physician for a medical examination and complete hearing evaluation to determine your need for a hearing instrument. There can be underlying medical issues that cause hearing loss that can only be diagnosed and treated by a physician. It is in your best interest to see a physician. The FDA allows you to waive this requirement by completing the
Medical History & Medical Waiver form.*
*Please note that based on your audiogram and/or medical history information it may be necessary for us to refer you to a physician prior to the sale of your hearing aid. This is mandated by state and federal laws.
HIPAA Privacy Policy: In the Health Insurance Portability and Accountability Act (HIPPA) the FDA mandates that your health related information is kept private. It is necessary for you to review our policy and sign that you are in agreement with the contents. Click here to access this HIPAA form.
These forms can be completed on-line by clicking on the links above. You may also print, complete and mail or fax these forms to us, along with your audiogram.
Step 3: Select Appropriate Hearing Aid Model
In determining the appropriate hearing instrument solution for you, our hearing aid professionals will take into account your audiogram, lifestyle and personal preferences.
Each hearing aid is manufactured with a specific fitting range, which means that each style is appropriate for certain, but not all, types of hearing losses.
The results of your audiogram will be compared to the fitting ranges of our digital hearing aids to determine which of these will best meet your specific needs. We will review your options with you and help you pick the model that will best meet your needs now and over the next few years, should your hearing needs change during the life of your hearing aids.
Step 4: Place Your Order
Following completion of Steps 2 and 3, America Hears hearing instruments can be purchased with a call to our Customer Service Center. RITE and OF styles can usually be shipped within twenty-four business hours of receipt of your order. Custom hearing instruments, and instruments that require a custom earmold are built and shipped usually within one week following receipt of your order and impressions.
